Examination Agreement Form for those who register for the exam
Every candidate who takes the certification examination will be required to agree to the terms set forth below at the time of the examination. After signing this Agreement, either online or in the paper-and-pencil administration, the candidate will be allowed to proceed to the examination.

• I attest that I have read and will comply with all the rules and regulations for the administration of the certification examination.
• I will comply with the Board of Certification policy to keep confidential all information about the content of this examination.
• I understand that passing the certification examination is not equivalent to possessing certification and does not entitle me to use the CO-OP^SM certification credential. I understand that if I use the CO-OP^SM certification credential before having been formally awarded certification by the Board of Certification, I may be ineligible for certification in the future.
• I agree that any disputes that are not resolved through the stated policies and procedures of the Board of Certification will be resolved exclusively under the law of, and in the courts of, the state in which the Board of Certification's offices are located.
